Super Smash Bros. Brawl

Attachment Kirby has an unused animation that appears to be him walking with a hammer and then swinging it with full force in the same manner as King Dedede's Jet Hammer move. Kirby charging up and releasing his hammer is something he would later be able to do in the next game of the series onward.
Team Fortress 2

Attachment In December 2009, as part of the WAR! Update, Valve ran a competition between the Soldier and Demoman classes. The objective was to kill as many of the opposite class as possible, with reward going to the victor.

At the end of the 8 day event, it was revealed that the Soldier was the winner with over 100,000 more Demomen killed than Soldiers. As a result, the Gunboats were added to the game as a secondary item for the Soldier.

For years it was speculated what would have been added if the Demoman team won, until lead designer Robin Walker replied in an email: "It would have been the Gunboats. It was an item we'd tested with both, and seemed to work fine with either." -- Robin Walker
Crash Twinsanity
1
Attachment A level called "Megalomaniac Mind Mania" was cut at a very early stage of development. The Psychetron would have transported Crash into Dr. Cortex's Brainovial Hyperbarium to explore Cortex's twisted mind and discover the connection Cortex has to the Evil Twins. Every enemy character was an ugly version of Dr. Cortex and would have driven around in tanks. The whole level was to be a musical number, sung by Dr. Cortex called 'Take a Walk Through My Brains'. Lyrics for the song were written, but ultimately after the level was cut, went unused.
Grand Theft Auto IV

Attachment An MP5 was originally going to be the SMG as seen in early pre-release screenshots. It had a 15 round magazine. An image of Niko holding it confirms this. Niko would presumably fire it with both hands on the pistol grip, like the Micro SMG. Its design was later replaced to resemble the S&W MP10.
Team Fortress 2

The idea for the Spy came from a bug which caused some enemy players to appear as allies.
Gex
1
Originally, the game was about a stunt man named Gecko X, who worked for a studio that was in financial trouble due to the main antagonist, Karl Chameleon. Each level would be themed around a Hollywood action movie genre, which would show stock footage of old Hollywood movies at the beginning and allow Gex to run around and do "stunts". The better the player did in the level, the more money the "Movie" made and therefore the better the studio did.

The idea was dropped because the developers found it hard to base a game around a real world theme while still making it believable and fun, compared to Mario and Sonic, which were based in fantasy settings and therefore nothing that appears in the game needed to be explained. The change in plot and setting to 'TV Land' allowed the developers to have this fantasy setting and some freedom to do whatever they wanted.
The Legend of Zelda: Ocarina of Time

Attachment The game features a crash debugging tool that allowed the developers to find out information as to why the game has crashed. This is still present in all the Nintendo 64 releases, and can be accessed by inputting a series of button combinations after crashing the game. Information about the game and the environment that was present during the crash will be accessible from this screen.

The Version Info screen, however, not only has information about when the game's version was compiled, but also a message which reads, "I LOVE YOU".
Grand Theft Auto: Vice City
1
On Rockstar's official Kent Paul website, the name "BJ Jones" is present on a list of characters from the game. This likely refers to BJ Smith, and could suggest that "Jones" was Smith's surname during development.
Grand Theft Auto: Liberty City Stories
1
Attachment A pre-release screenshot showed the Avenging Angels wearing red berets instead of black caps.
Grand Theft Auto: Vice City

Attachment In early screenshots and trailers, the Pump Action Shotgun had the same model as the Pump Action Shotgun from Grand Theft Auto III, but it was later changed.
Godzilla: Save the Earth

The plant monster Biollante was intended to be a playable character, but was cut late in development. According to Simon Strange, one of the developers, she was complete, but perceived as being "behind schedule", and the final version had not been cleared with the license holder, Toho. She still exists within the game's code, but was made inaccessible.

"I believe she was cut for political reasons, probably stemming from the bad blood that developed between Pipeworks and Atari" - Simon Strange
Quake
1
Quake was first conceived as an adventure or RPG game, similar to The Legend of Zelda. Quake was to be the name of the player character, and he would wield a massive hammer, which would increase in power as he defeated enemies. By the time the development team had completed the engine and networking code, they were burnt out on the project. Uncertain that they could meet their release date, they held a vote and decided to change the gameplay to something they were familiar with: a first-person shooter, like their earlier release Doom.
Call of Duty: Black Ops II

Attachment The Predator reticle for the Reflex Sight was inspired by the Predator's laser sight from the Predator films.
Grand Theft Auto: Vice City

Attachment Concept art on Rockstar North's website shows that the working name for Ricardo Diaz was "Fernando Diaz".
Grand Theft Auto: Vice City

An unused phone conversation between Tommy Vercetti and Avery Carrington mentions an unused area called "Gator Keys", and suggests that there would have been a mission to go there.
Bravely Default: Flying Fairy
1
Attachment The Western releases of the game were censored, with several outfits made less revealing and the ages of some characters were raised from 15 to 18.
Beyond: Two Souls
1
David Cage was inspired to write the story of Beyond: Two Souls after a member of his family passed away. He wanted to write a story about death, but not from the usual perspective, and treat it like a physical event that can be explained.
Crash Bandicoot: The Wrath of Cortex
1
The game actually started out under the direction of Mark Cerny (producer of the past games) exclusively for the PlayStation 2 and was initially intended to be an open world adventure. After Universal Interactive (the holder of the IP) decided to take the series multi-platform, and have Traveller's Tales develop it, the decision was made to use the more traditional linear path level design.
City of Heroes
1
Attachment The NPC superhero Coyote, who appeared during the in-game tutorials, was based on the player Mathew Bragg, who went by the alias Kiyotee and had passed away soon after the game's release. Bragg was actively involved in the game's community and often welcomed and assisted new members who had joined.
Skylanders: Spyro's Adventure

Attachment An early model for Sunburn shows his figure was to wear a crown, possibly linking him to royalty.
