Night Warriors: Darkstalkers' Revenge

In the Japanese version, beating the game yields a character ending, followed by a short epilogue with the player character's portrait scrolling in the background, and finally the ending credits. In all other versions, the epilogue was cut, leaving only the scrolling picture.

Attachment The game's original title was 1080° White Storm, similar to Wave Race: Blue Storm. This can be seen in the game's files, where an unused placeholder title screen can be found.
Sonic the Hedgehog

Despite going through the same parts of Flame Core at the same time, Sonic and Shadow are not playable in each other's episodes. However, there are unused sound files that suggest that they were going to team up, with Shadow driving a vehicle while Sonic takes out enemies.
Buzz Lightyear of Star Command

An unused audio file called "buzvbuz.wav" can be found in the game's files. The file is a clip of Bean Bunny from The Muppets.
Darkstalkers: The Night Warriors

There is evidence in the code of the arcade version that the two boss characters, Huitzil and Pyron, may have been planned as playable at one stage of development. There is an alternate color scheme for both of the characters that shows up if you are playing a mirror match, both characters are able to be played in Arcade mode though hacking (though Pyron can't fight the final boss due to a bug), both of the characters have their own endings, which are the exact same as their endings in the sequel, "Night Warriors: Darkstalkers' Revenge", though in a more unfinished state with the ending text just repeating "A" and the music not playing like it should.
Mortal Kombat: Shaolin Monks

A hidden survival mode can be found in the foundry stage. It was apparently never finished, and was removed from the main menu. However, it can still be accessed in-game through a long and complicated procedure.
CarnEvil

Attachment Inside the game's files is an unused image of Jeffrey Dahmer holding a paper bag with the head of Neil Nicastro (executive producer, and CEO of Midway Games until 2004) in it. The image appears several times in various sizes, and was most likely used to fill empty space in the game's data.
Dark Souls

Unused text from the game's data reveals that Shiva of the East's main goal is to obtain the Chaos Blade. If the player manages to get one, Shiva will even go as far as attacking the player to obtain it.
Crash Twinsanity

A level called 'Gone A Bit Coco' that took place in Coco's mind was cut late in development because it was glitchy and incomplete. It can be seen in early gameplay videos, and the level music was used in the credits sequences.

According to executive producer Takashi Tokita (scenario writer of the original release), 3/4 of the original script had been cut from the Super Nintendo release due to hardware limitations, but some of this lost material was reworked into the DS remake.

Valerie Bourdeau stated that the developers removed quick-time events, as shown in the original E3 trailer, due to the negative feedback that the gaming community gave.
Mario Kart: Super Circuit

Attachment There are Tilemaps and Minimaps for all of the battle courses from Super Mario Kart. While these might have only been used as placeholders before the new ones were completed, it may also have also been possible to unlock these courses just like the tracks at one point. These are all stored in one piece, unlike the rest of the tracks and battle courses, which are stored in chunks.
Sonic Adventure

In the Dreamcast version of Sonic Adventure, you can explore under the deep water in the Egg Carrier Chao Garden. Under the water lies a secret ring far out of reach. This was removed in the DX update, because the depth of the water was reduced, probably to prevent Chao from swimming out of reach and getting stuck in the terrain.
Grand Theft Auto: San Andreas

There are unused files for the M249E2 SAW which was originally intended to be included in Grand Theft Auto: San Andreas, but was never actually in it. They can be found in the PS2 version.
WWF No Mercy

Attachment The Big Show was originally intended to be featured in the game, however he was removed during development due to his contract expiring. However, using a Gameshark, it's still possible to access his face texture in Wrestler Edit mode. The name of the face texture is RROR (for ERROR).
Harvest Moon: Magical Melody

Attachment The Gamecube version of Magical Melody gives the player an option to begin the game as either a girl or a boy.
This gameplay feature was removed from the Wii port, so that only the male protagonist is playable. Any reference to the female protagonist was also removed from the cover art.
Dragon Ball Z: Budokai 3

There are several unused voice clips that suggest that there were going to be more characters in Dragon Universe than there actually are. Notably, Kid Goku's Dragon Universe seemed to have involved him finding a time machine that takes him to the current Dragon Ball Z era, fighting Yamcha, Krillin, Tien Shinhan, Piccolo (who he thinks is still King Piccolo), maybe Vegeta, Teen Gohan (who thinks he stole the name from his grandfather) and Broly.
Activision Anthology

Two unused games, Rampage and Skate Boardin', are in the game's files. They still have their cartridge artwork and TV screens, and can be played by renaming some files on the disk.
Star Wars Galaxies: An Empire Divided

Attachment There are a number of items and objects that were found in the game's files which went unused. These include paintings, posters, an Imperial Space Station, creatures, droids, and the planet Taanab (first mentioned in the Star Wars Episode VI: Return of the Jedi novel) which was originally considered to be a planet to explore, although it remains unfinished.

Attachment Inside the game's files is an unused preview image of the Adidas Brazuca, the ball for the 2014 FIFA World Cup. However, the ball uses the texture and model of the Adidas Cafusa.
