Injustice: Gods Among Us
1
Attachment Nightwing's Regime costume is Damian Wayne. This makes Nightwing one of several characters to have different people as alternative costumes. Spoiler:It is revealed during the game that the alternate dimension version of Damian killed Dick Grayson, the current true Nightwing, took his mantle and joined Superman's regime.

The other characters with different people as costumes are Green Lantern (with a John Stewart costume) and Superman's Cyborg outfit (which is actually a character named Hank Henshaw.)
Duke Nukem Forever
1
During the first fight with an Assault Commander boss, Duke says, "I'm going to kill you old style!" This is a reference to the Youtube video Ventrilo Harassment where a Ventrilo user is harassed by someone with a Duke Nukem soundboard. The voice actor for Duke Nukem, John St. John, has mentioned in interviews that he has seen the Ventrillo Harassment video, which probably lead to the line being in the game.

"It's awesome, especially the Ventrillo Harassment! I LOVE that fans have used my voice to create some very funny videos and soundboards."
Sonic Unleashed

Attachment The term "Werehog" is derived from the words werewolf and hedgehog. However, this is a misnomer, considering that the "were" in werewolf derives from the Old English "wer", meaning "man", which means he is incorrectly referred to as a "Manhog". A "Manhog" would be a man who can morph into a pig, or someone who is half man, half pig.
Franchise: Crash Bandicoot
1
The concept of Trash Bandicoot (now known as Fake Crash) came from some cheap looking knock off Crash Bandicoot dolls. These dolls had huge eye brows and a giant overbite.
Star Wars: Rogue Squadron
1
Attachment In the Nintendo 64 version, entering the passcode "CHICKEN" will allow the player to pilot an AT-ST in a special bonus level. However in the PC release, entering the passcode will allow the player to pilot the Walker in any of the game's missions.
Minecraft: Java Edition

Attachment Gears were a type of block that were available in Indev 0.31 which could be used in a similar way to redstone. They could only be placed on the side of blocks, and were never available in the release versions. In earlier builds players could hack the game to make them accessible, but the files were removed in later updates.
Super Smash Bros.
1
Attachment In the character select screen, the characters (excluding unlockable characters) are placed in the order of when they first appeared in their respective titles, starting with the oldest, Mario and Donkey Kong, and leading to the most recent, Pikachu.

Attachment A quote from Super Smash Bros. Melee offers an origin to the Like Like's name:

Supposedly, their name comes from an old, almost indecipherable Hyrulian proverb: "Shield-eaters and world leaders have many likes alike.

The Legend of Zelda: Ocarina of Time

Attachment The name of Link's horse, Epona, when translated from Gaulish means "Great Mare" and is the name of the Celtic God of horses, donkeys and mules.

Franchise: Mario
1
Mario's hair is brown, while his mustache is black. This was because around the time of Super Mario Bros. 3, Mario's creators wanted him to have a definite hair color in game, while it was easier to draw his mustache by extending the black outline of his nose.
Mana Khemia 2: Fall of Alchemy
1
The English version of Mana Khemia 2's title is actually a mistranslation. Mana Khemia 2's Japanese title is "マナケミア2 〜おちた学園と錬金術士たち〜", or "Mana Khemia 2: ~The Fallen Academy and the Alchemists".

The Japanese title is also more reflective of the game's plot, as the story has more to do with an alchemy school's decline than it is with the decline of alchemy itself.
Ar tonelico: Melody of Elemia

Attachment After you clear Misha and Aurica's Cosmospheres, you can access the special 'Level E' world. These worlds are based on the games Atelier Marie and Atelier Elie, respectively. After you complete the Cosmosphere, Aurica will unlock the 'Marlone' costume, and Misha the 'Elfir' costume, which are based on the Atelier heroines' outfits.

Finally, while not an unlockable costume, there is art of Shurelia in Lita's outfit from Atelier Iris; you can unlock it by getting one of her endings.
Mega Man X6
1
Metal Shark Player's Japanese name is actually Metal Shark Prayer. His English name is the result of a mistranslation of R and L being identical in Japanese. The reason he is called Prayer is because he resurrects other Mavericks (these being Sting Chameleon, Magna Centipede, and Blast Hornet), thus he "prays". It is also worth mentioning that when he resurrects the Mavericks, he shouts their Japanese names.
Metal Slug 5
1
Attachment In an unused debug level, you can find an assist character named Acchiles. He was left out due to Metal Slug 5 being rushed. In one of Metal Slug 5's flyers, he is seen along with an unknown character.
Batman: Arkham Asylum
1
At the end of the game, there is a crate of TITAN floating in the water. A randomly chosen villain (out of Bane, Scarecrow and Killer Croc) will grab hold of the crate just before the fadeout.
Resident Evil
1
The Saturn version of the game also added a second Tyrant. After the player's first victory over the Tyrant that Wesker unleashes, when attempting to leave, another Tyrant will burst out of its stasis container.

In Ratchet & Clank: Going Commando, there is a badly programed enemy on planet Grelbin called the Snow Beast. The enemy was made in three days and has too short of a pre-attack animation, which makes it frustrating to fight, especially in large groups. This beast was the influence for the Snow Beast Award, which is an internal award that Insomniac Games gives to the person that "Created the worst thing that actually shipped in the game" after every new game is released.

It should be noted that in the final game these 'Snow Beasts' are individually named 'Y.E.T.I.'
Mega Man X

If you use the boomerang cutter when fighting Flame Mammoth you can cut off his trunk so he can't shoot the oil slicks.

Platform: Philips CD-i
2
When the Phillips CD-i was first introduced in 1991, it was not advertised as a gaming console, but as a multimedia product to promote the CD format, aiming to provide an all-in-one experience with a single system. Consequently, in addition to audio CDs and game discs, the CD-i included support for the following sub-formats:

• CD+G discs, a variant of audio CD with allows listeners to display low-resolution graphics on a monitor in time with the music.
• Video CDs, a precursor to the more successful DVD format, distinct from both CD+G and the CD/LaserDisc hybrid format CD Video. Video CD playback required the installation of a supplemental MPEG-1 decoder cartridge.
• Photo CDs, which allowed users to store and display uncompressed digital transfers of film photographs on a video monitor.

Low sales caused Philips to try promoting it as a game console three years later.

Pokémon Crystal Version
1
Attachment Even though Pokémon Crystal is incompatible with the Game Boy, Game Boy Pocket, Game Boy Light, Super Game Boy and Super Game Boy 2, there is an unused Super Game Boy border programmed into the game. This can be seen by setting a flag in the game's RAM to enable the Super Game Boy features. Cheating devices such as the Game Genie can also be used to achieve the same effect temporarily. The border is a leftover from the Japanese version of Pokémon Gold; the design is the same and the text reads Pocket Monsters Gold Version, rather than the English version's title of "Pokémon Gold".

When the player attempts to play the game on a model of the Game Boy prior to the Game Boy Color, the error message "This Game Pak is designed only for use on the Game Boy Color." is output by the game, with the word Crystal spelled out using Unown. There is an unused color palette associated with this message, using an orange tint, which appears when triggering the unused Super Game Boy border.
