When Pokémon Red, Blue, Yellow, Gold, Silver and Crystal were released on the 3DS Virtual Console in Europe, they were given a PEGI 12+ rating due to the playable casinos in Celadon City and Goldenrod City.
Mortal Kombat 3
1
Attachment The Game Boy version of Mortal Kombat 3 was released in North America in 1995, one year after the formation of the Entertainment Software Rating Board (ESRB), and became the only original Game Boy game to receive a Mature 17+ (M) rating.

Wario's Woods
1
The game was the last NES game to be released in the US on December 10, 1994. Because of the release date, it's also the only NES game to receive an official ESRB rating, being "K-A" (Kids to Adults).
Fallout 3

The drug "Med-X" was originally just called Morphine, but in order to get a MA15+ rating in Australia by the Office of Film and Literature Classification (OFLC) board, all references to Morphine had to be changed to "Med-X". During the time there was no higher age rating (R18+) for video games, which would have meant the game could not be sold in Australia. The reason the OFLC board didn't pass the game was because of the usage of real-life drugs.

Mortal Kombat
1
Mortal Kombat is partly responsible for the creation of the ESRB; mostly due to Sub-Zero's head rip Fatality being so violent.
Spider-Man
2
Attachment If you put in the cheat code "girlnextdoor" you can play the game as Mary Jane Watson. At the end of the game, where Spider-Man and Mary Jane usually kiss, Mary Jane will kiss herself. This code was removed from later versions of the game, presumably because of the lesbian kiss in an E-rated game.
Donkey Kong 64
1
Attachment The Kong's weapons were originally real guns, as opposed to the comical fruit and nut based weaponry seen in the final game. This was changed in response to Shigeru Miyamoto being horrified by them upon paying a visit to the Rare headquarters to check on the game's development. He then sketched out and offered a replacement that would be more fitting to the world of Donkey Kong, which would ensure that the game would receive a child friendly rating.
