The PC version of Halo 2 was delayed from release due to an image of a man, who was one of the developers, mooning the camera and exposing his bare buttocks in the game's data (in an error report known as .ass).
Asheron's Call
1
Attachment A Police Box would randomly appear throughout random locations of Dereth, a reference to the TARDIS from the science fiction series "Doctor Who". However after the release of the second expansion pack "Throne of Destiny", it was removed from the game due to "various reasons" according to the developers.
Banjo-Tooie

Attachment In 'the Workers Quarters' of Grunty's Industries, you can find Loggo in the men's bathroom, who has something clogged inside him. If you talk to him for a second time before unclogging him, Kazooie will bring up the option of calling a plumber and mentions 'Mario' by name. In the XBLA version, the reference is changed to "that well known Italian guy" as Microsoft could not use Mario's name.

The first time is when Twinrova announces that the three flames are now lit, saying that the Flames of Sorrow and Destruction were lit by "Veran and Knox". Later when battling Twinrova, she announces that she will "not fall like Knox or Veran". Curiously, this does not occur in Oracle of Seasons.

1
In the original Japanese version, if the player runs to where a key will land after defeating Bowser in the Dark World or Fire Sea, presses C-Up and looks somewhere, Mario will keep looking in that direction during the key collection cutscene.
Zool
1
Attachment In the ending to the Amiga version, once Zool gets back to his planet there is a blue hedgehog with red shoes that walks across the screen before being kicked out of sight by Zool running by. This was meant to be a shot at Sega's Sonic the Hedgehog, which Zool was meant to be in direct competition with.
Metal Gear Solid V: The Phantom Pain
1
Attachment There are many differences between the original "Metal Gear 2: SOLID SNAKE" on MSX and the version included in "Metal Gear Solid 2: Subsistance". In the 2006 version of "Metal Gear 2: SOLID SNAKE" they changed the portraits in the "codec calls" with Yoji Shikawa's artwork, and they also added a new line in Gray Fox's speech after you defeat him:

In Japanese
その時、俺は耳も鼻もそがれ。。。ひどい状態だった

In English:
"They had cut off my ears and my nose, I looked terrible"

These new words fit perfectly with Ishmael's bandaged face in Metal Gear Solid 5.
Halo 2
1

Secret of Mana
1
In the NTSC version, there is actually a way to obtain the Mana Sword before the final battle without the use of a cheat device. First, once the Flammie Drum is obtained and the sword is level 8, go to a small isle of the coast north of Ice Country due west of the Ice Palace. Neko will be there. The player should save at Neko, soft reset and start a new adventure. When the player enters the Mantis Ant fight, perform a soft reset once Elliot finishes his lines and reload the save file in Ice Country. Instead of Ice Country, the entire party will be in the Mantis Ant fight. Defeat him and Jema will give the party a Sword Orb. Give this orb to Watts to forge and the player should have the Mana Sword.

As such, most of the game's bosses were changed to characters from the story, with the exception of Captain Hook.

In the Master Quest version of the game, the Water Temple has an extra key that cannot be used. This is due to the fact that a flag in the game's code is accidentally set when Link defeats the three Stalfos in the room with the three dragon heads. This was fixed in the 3DS version.
