Attachment The texture for the Skull Kid assist trophy contains an unused portion which seems to be the flute he used in Ocarina of Time and Majora's Mask. It is unknown what it could have been used for, though it's likley he was supposed to use it in some way when the player summons him.

Handsome Jack had dialogue recorded for the New-U Station revival taunts, but they were unused in the final release.

Attachment An unused version of the Phazon Suit can still be found in the games files, which takes a more blue/turquoise color scheme as opposed to the final black-and-red version.
Senior Artist Mike Sneath commented on it, saying:

"Yes you are right that is what was another version of the Phazon Suit which was done by another artist Gene Kohler. Gene made all the suits with the only exception being the black phazon suit. That suit you mentioned was cut because if I remember the Art Lead Todd Keller didn't feel the suit felt evil enough. That was the whole idea about liquid Phazon is that it transforms things into being aggressive and destructive and Todd wanted that to be reflected in how the suit look. That is all I can remember with regards to that suit you mentioned and I hope I got the story right on that as it was a long time ago but I believe that is why it got cut." [sic]
The Legend of Zelda: Twilight Princess

Attachment In early trailers of the game, Midna had blue and orange hair instead of the solid orange hair visible in the final version. Also, Link had the ability to dive as if he had equipped the Zora armor while wearing his regular tunic, but his regular swimming abilities are limited in the final game.
Demon's Souls

Attachment There is an unused model of a child in the game, who is speculated to be the child of the Filthy Vendor from World 5 (Valley Of Defilement). The child has no included animations.
Mario & Luigi: Bowser's Inside Story

In Mario & Luigi: Bowser's Inside Story, there are quite a few unused rooms without anything in them, most of them used for testing. One of the rooms appears to be a pipe in Dimble Woods leading into bowser's body that can only be accessed through the use of hacking. It was probably removed because once you've found all the attack pieces there you had no need to go back. Another one of those rooms are in Dimble Woods, along with what appears to be the Tower Of Yikk on the ground. This might suggest that originally bowser was supposed to fight it in dimble woods, but it was replaced with Bowser's Castle.

Claus has over 150 unused sprites, the most out of all the characters in the game, including full running and walking animations in a bathing suit.
Super Mario Sunshine

Attachment There is an unused enemy named "Kug" found in the games files. Although there's no way to access it normally, it can be found under Pinna Park by glitching the camera, or by using a cheat device.
Borderlands

Within the game's files is a video file that was likely a trailer for the game. Never shown before or after the game's release, the video features the song "Greenfields" performed by the Brothers Four, as well a few small differences from the final game such as an early model for the character Lilith, and "Fyrestone" being spelled "Firestone".
Donkey Kong Country

Attachment The "Puftup" enemy from Donkey Kong Country 2: Diddy's Kong Quest was originally intended to appear in this game, and its idle animation is even found in the game's code.
Dark Souls

Attachment The Crystal Lizard model from the previous From Software game Demon's Souls can be found within the game's files. It was likely used as a placeholder until a new model was made specifically for Dark Souls.

Attachment Inside the game's files is an unused soccer ball. It's likely this could've been used for testing the models, or it could've been used for one of the Wuhu Island stages.

Attachment There are unused sprites of King Dedede in Kirby Mass Attack's memory. These sprites are shown from above, suggesting that he was intended to be a playable character in the Strato Patrol EOS sub-game at one point in development.
Maya the Bee & Her Friends

Attachment The game was originally supposed to be a South Park game, but series creators Matt Stone and Trey Parker did not want to license an adult game for the Game Boy Color, so it was changed to a Maya the Bee game. Some South Park graphics still remain in the game's ROM.
Castlevania: Symphony of the Night

It was thought that, as the game takes place in Dracula's castle, Alucard should have his own room located at the top of the tall tower found to the right of the map. Within his bedroom would have been a character called the "Skeleton Carpenter", and as the game progressed the room would be built up with new furniture. There were thoughts of also including items and decorations throughout the game that the player could pick up and use to decorate the room. This feature was later cut from the game.
Castlevania: Symphony of the Night

During the game's prologue, in which text scrolls up the screen explaining the story leading up to the events in Symphony of the Night, narration was recorded to be played over the top of the text in the English release just as there is in the Japanese release. This was removed prior to release, as Konami's PR team felt the recording was bad, and detracted from the scene.
Disney's DuckTales

The Transylvania theme was completely different in the game's prototype, and the moon theme was originally 20% slower than in the final version.
Crash Bandicoot 2: Cortex Strikes Back

Attachment As it was shown in the E3 preview video, the "Ruination" level used to have a stone signpost showing the "easy" and "hard" paths through the level. Although the signpost has been removed in the final release, the code and data for it still remain in the game's files and are completely functional.

In N. Gin's boss level, there are 20 missile paths in the data, even though only 10 were used. A lot of these unused paths are also misplaced and can't hurt the player.
