Metal Gear Solid: Snake Eater 3D
1
Spoiler:If you have the 3D on, when you go into the first person view shots, they'll be in 3D. However, after Snake gets his eye shot out, first person view shots that look through Snake's eyes aren't in 3D. This is because he lost his right eye and has no depth perception, thus players cannot see in 3D.

The Japanese/European soundtrack for the game is different compared to the American version. An example of this would be the intro and ending songs. In the Japanese version, the intro song is "You Can Do Anything" while the ending song is "Believe In Yourself" by Cosmic Eternity. In the American version, both the intro and ending songs are "Sonic Boom" by Pastiche. Because the American version got a new soundtrack and the intro and ending FMV sequences were being re-edited to fit in time with the respective music, the game was delayed a few months.

Attachment In the level "Dead City", the creatures known as "Dark Ones" can be seen at various points throughout the level, watching Artyom. The first one can be briefly seen in the offices, the second one will cause a flashback when Artyom passes through the playground, and finally the third one is checking a corpse in the gated passage to the courtyard of the building Bourbon told Artyom to head to. Shooting one will get you a negative "moral point".

Trials of Mana

Attachment There seems to be a planned boss in the place where the party meets Jagan for the first time in Bucca Island that was removed prior to release that can only be accessed by using Action Replay on sample cartridges of the game. The boss's body is submerged under the water and attacks with two tentacles.
Half-Life 2: Episode One
1
Attachment There is an unused weapon with the file name of "#HL2_Hopwire". The weapon sucks up everything in its path into some sort of warp. It's not known how it would fit into the story, but it would probably have been used against Striders, based on some console codes that mention the weapon. It can only be accessed though console commands.
Naruto Shippuden: Ultimate Ninja Storm Generations
1
Attachment In Naruto Shippuden: Ultimate Ninja Storm Generations, if you enter Free Battle mode, select "Versus Battle", select the "1P vs. 2P" option, complete one match with supports, go back to the Free Battle mode selection screen, select Training mode, press B (or circle for PS3) on a support-only slot, you will be able to play as them. They have no Awakenings, B (or circle) combos, or voices and can only use their normal jutsu, powered chakra shurikens, normal dash, guarding, and normal walking. Attempting to do a side ninja dash (Left or Right on the Analog Stick and double A or X) will lock up your player unless you press your D-pad either left or right. Even though they are in an unplayable state, it suggests that all of the support-only characters were going to be fully playable since under normal circumstances support-only characters don't use their walking animations.
Dark Souls

The Ceaseless Discharge, who is revealed to be the only son of the Witch of Izalith by the in-game description of the Orange Charred Ring, can be instantly killed if provoked into attacking the player after they remove the Gold Hemmed set from the altar near him. Provoking him into attacking the player there specifically and then running back to the Fog Gate makes him charge after the player and leap over the chasm below to try and stop them from escaping. The result leaves him desperately grabbing the ledge, from which point the player can attack his arm and send him falling to his death.

Though difficult, it is possible to defeat the Vanguard at the end of the tutorial stage. Doing so will lead the player to a room within the Dragon God's lair in the Stonefang Mine, where several Grasses and Stones lay as a reward. However, after going as far as they can a cutscene ensues where the Dragon God kills the player in one blow, setting in motion the events that would have happened by dying at the Vanguard's hands instead.
Dark Souls

Despite the name, the Four Kings fight can produce more or in rare cases less than Four Kings. This is due to the nature that the boss fight itself is one health bar shared between however many kings required to empty it, with a new king spawning every minute there is less than four. Because of this spawning, if a player kills three kings and fails to kill the fourth fast enough, three more kings can eventually spawn with full health on the individual monsters, but no additional health added to the total health bar. The health also seems to increase when Phantoms are summoned to assist the player.

On the flip side, under the right conditions there can be as little as ONE king spawned that still results in victory for the player. Kings cannot INDIVIDUALLY be damaged when performing their Lifedrain grab, but attacking a king in the middle of the animation does damage to the total Kings lifebar. They are still able to be damaged during their death animation as well, and with enough players and the proper buffs, this makes killing less than four of the Kings possible.

Secret of Evermore
1
Attachment If the player waits for several minutes after "THE END" appears in the credits, an additional scene is shown wherein Professor Ruffleburg approaches a docile Carltron and says, "That's a good robot, Carltron. Dust, dust. Clean, clean. And no more plans for world domination, OK? Good." After Professor Ruffleburg leaves, Carltron turns to the camera with an evil grin. This is followed with a teasing, "THE END?"
Secret of Evermore

Originally, the title for "Secret of Evermore" was simply, "Evermore." Squaresoft decided to add "Secret of" only a month before the game shipped, hoping that title familiarity with the already released "Secret of Mana" would benefit sales.
Secret of Evermore
1
Attachment If the player waits for approximately thirty minutes after "THE END?" is on screen, an additional message appears, stating, "You're a patient one, aren't you? Well, here's another cryptic credit, just for you..." Followed by the lone credit, "DOLLY GRIP Brian Fehdrau." This is a joke credit attributed to lead programmer, Brian Fehdrau.
