Sonic Generations

Attachment The Sonic Generations level list was conducted almost entirely on fan feedback, unknowingly to the fans themselves. Between the game's development cycle, SEGA (specifically the "Sonic" Facebook page) released a poll back in 2009, along with other unimportant ones, simply asking the reader to list their favorite levels from each Sonic game, leaving fans none the wiser. The most mentioned levels from both fans and some internal employees became the official level list for Generations.
Final Fantasy X-2

According to Motomu Toriyama, director of Final Fantasy X-2, the idea for three playable female charters came from Hollywood movies like Charlie's Angels and Tomb Raider.
Super Princess Peach

Attachment The Koopalings were originally going to be a part of the game, but were removed for unknown reasons. Sprites for of all seven of them have been found, with Roy's shell being miscolored as green instead of pink.
Left 4 Dead

Attachment During development, there was another Special Infected called the Screamer, when attacked or agitated, the Screamer would run to a safe location and yell out a loud Scream, calling out a horde of zombies. While it was cut, many things were retained from it. The idea of attracting hordes was given to the Boomer in the form of his Bile. The concept of having to avoid an enemy completely or suffer the consequences was given to the Witch. His scream was given to the Hunter, and his maniacal cackle was given to the Jockey.
Sonic the Hedgehog

Attachment The game was originally envisioned by SEGA to be a reboot to the entire franchise, and to build the game similar to the Adventure series, fearing that the previous "Sonic Heroes" and "Shadow the Hedgehog" had made the series stray too far. This also explains the realistic art direction that the game took, along with certain character interactions that don't make sense.
Pokémon Yellow Version: Special Pikachu Edition

Data exists for a battle situation when the Trainer has no Pokémon, though it isn't actually used when entering a battle with no Pokémon in the final release. In this case, selecting any option other than Run shows the message "Hurry, get away!" This may have been a prototype for the initial battle with the wild Pikachu at the start of the player's adventure, as the player has no Pokémon at that point. It could also have been planned for a possible battle against a wild Spearow with Pikachu unable to battle, making it closer to the events of the anime.
Portal 2

The Turret Opera, the song that was played at the end of the game, was sung entirely by Ellen McLain, who voiced both GlaDOS and the turrets. When recording the song, Ellen was asked to make up some words to put in the song, which she did using her "very bad high school Italian."
Mafia II

Attachment In the Japanese version, the collectible Playboy centerfolds were censored, even though the game was rated "Z" for 18 and above. However the magazine itself in Japan, which ceased publication in 2009, was never censored.

Pac-Man's name was changed from Puck-Man because arcade operators feared vandals would alter the letter P to F on the arcade machine.

Attachment Originally, the Boomers bile was red, and he was meant to have no "puking" capabilities. Instead, he was to explode onto the survivors, dealing significant damage. This was later changed to incorporate the "Screamers" (a cut special infected type) horde-attracting abilities.
Persona 3

Koromaru is likely based off a real dog named Hachikou. Both dogs owners died, but the dogs go to the place they last saw their master, to wait for them to return.
Kameo: Elements of Power

Kameo started out as a Gamecube game, and was meant to be a launch title. During the development, Microsoft bought Rare, and the title was moved over to the Xbox. After many delays, the title was moved to the Xbox 360 as an exclusive.

Sonic and the Secret Rings started out as a port of Sonic the Hedgehog (2006), but since they needed a game for the release of the Wii and they wanted to focus on the Wii controller, it was decided to make a brand new game.

The stage fatality was left out of "Pit II" because it would require a lot of work to create falling animations for non-MK2 characters.

Attachment In the official wallpaper for the game, Stryker can be seen in his MK3 outfit, even though it was never shown in-game.

In the Freedom HQ, you can find a series of screens that have some Precursor text scrolling on them. When translated, the text becomes a farewell message to Naughty Dog's pet/mascot, Morgan, who died at age 13.

Attachment With the success of Donkey Kong Country, Nintendo gave Rare permission for a sequel on the Virtual Boy. This happened just after the launch of the Virtual Boy and was only in the early planning stages for a matter of weeks before it was cancelled due to lack of sales of the Virtual Boy console.
