While the game's Project Giana Kickstarter campaign was active, voice actress Cristina Vee went to Black Forest Games' forums and offered to voice Giana for the game. While her offer was considered, voice actress Katie Leigh got the role instead.

There exists a bizarre occurrence in Bubsy: The Woolies Strike Back where, when pausing on certain stages, Woolies will transform into the demon enemy from Giana Sisters: Twisted Dreams, and then turn back when unpaused. It is unknown whether this is a glitch or an Easter egg.

In pre-release content related to Giana Sisters: Twisted Dreams, it was mentioned that the game would include a bee enemy that rises upwards. This enemy did not make it into the final game, but did eventually show up in another game using the same engine, Bubsy: The Woolies Strike Back, where it had a different behavior, being a directly hostile enemy that is distracted by flowers.

Inside the files of Giana Sisters: Twisted Dreams is a model of Ed Euromaus, the mascot of the Europa-Park theme park in Germany. When development studio Black Forest Games was asked why the file was in the game, they responded "We were young and needed the money."

Maria has an unused punk counterpart texture left in Giana Sisters: Twisted Dreams game's files. It is a recolor of Punk Giana, having red eyes, blue and green hair, a green sweater and a blue skirt. Its purpose remains unknown, though it possibly could have been meant for a co-op multiplayer mode or a scrapped cutscene.
