The Witcher 3: Wild Hunt
1
In the Blood and Wine expansion, there is a hidden achievement you can earn if you kill a frozen opponent with a crossbow bolt. The enemy can be frozen in two ways, by either using a Northern Wind bomb, or using the Piercing Cold to give a 25% chance of freezing opponents when Aard is cast. Beat down the enemy until they are low on health and then throw a bomb at them or try and freeze them with the Aard sign (keep in mind there is a chance they will die when doing this). Once they are frozen, finish them off with the crossbow. Alternatively, this achievement can be more easily earned on wild animals that can be killed with a single bolt. After the enemy is killed, Geralt will say "Hasta la Vista", and the achievement will be revealed, also named "Hasta la Vista™" but with a trademark symbol. This is a reference to a scene from the 1991 film "Terminator 2: Judgment Day" where the Terminator says "Hasta la Vista, Baby" before shooting the frozen T-1000.

Attachment Depending on what you name your save file, the name inputted will act as a seed that determines the position of the twinkling white stars in the night sky, while the blue stars and larger red planet remain in the same position. Since there is a character limit of 8 characters for a name, the number of possible variations is estimated to be somewhere under 5.3 billion, the key word being "under" as this cosmetic feature does not utilize all 127 characters available, so the actual number is bound to be lower. This feature was explained to the Japanese audience at launch, but this was not known in the West until 2021.

Attachment In Chapter 4 'Cuckoo Clock', during the level section 'Gates of Time' players can find a river boat. If players get in the boat and ride along the track, they will come across a house on a pier. If a player enters this house inside they will find numerous blue pots with white stripes & large crates. The pots can be broken and inside them are green Rupees. This room is a reference to the Guard House of Castle Town from The Legend of Zelda: Ocarina of Time, a location infamously used to easily farm Rupees. An Achievement/Trophy is also unlocked: 'Force Triangulated', with the description, "Breathe some wind into your ocarina, my skyward princess".
Super Mario Kart

Attachment The game has a secret CPU ghost mode which can be accessed under the Time Trial menu by pressing Start on a controller in port 2 while selecting a driver. This triggers a CPU cursor to appear, allowing the player to select a ghost driver to compete against. However, the CPU ghosts are capable of interacting with elements of a track's mechanics, like the ice blocks found in Vanilla Dome.

If the player allows the CPU to destroy these blocks, then drives over where the blocks were, then saves and race against their own ghost, a glitch will occur where the ghost will vanish when it touches the block that was supposed to have been destroyed and trigger a visual error where the player's tires will cycle between two frames of animation even while in an idle position. This glitch only occurs when the player and the ghost use the same driver.
Mortal Kombat II
1
In Episode 7 of the TV series adaptation of The Last of Us, Ellie and Riley are seen playing the arcade version of Mortal Kombat II. At one point, Riley uses Mileena's fatality on Ellie's character Raiden, with Riley being heard pressing four buttons to initiate the killing blow. Following the episode's release, a number of fans online claimed this was inaccurate, as one can simply just press and hold the Heavy Kick command during the "Finish Him!" prompt to initiate her Fatality. However, other Mortal Kombat players argued that the show's display of gameplay could possibly be correct, pointing out that there is an alternate way to do the fatality. Specifically, Mileena can perform the Fatality by standing close to the opponent, holding the Block button and tapping the Heavy Kick command.

Paper Mario: Sticker Star
1
In the scrap-filled area in Shy Guy Jungle, hidden among the scraps of trashed paper are the following two messages:

Thank you for your letter.
Sorry I'm not here to receive it, but I
will get back to you shortly. Thanks.
Parakarry

"Observations on the Ancient
Civilization of the Chomp Ruins"
University of Goom
Goombella

Parakarry was a party member from Paper Mario, while Goombella was a party member from Paper Mario: The Thousand-Year Door. These are the only two references to earlier Paper Mario titles in the game.

Additionally, a third message can be found in the trash heap which simply reads "XD3R-B8HH-9ZR2-FL16". Despite resembling a Club Nintendo or Nintendo eShop code, it registers as invalid when you attempt to input it on either service, and no further information emerged over the years regarding its significance.
